Where to Send Your Completed Agreements Questions



EUROPRACTICE member organisations wishing to gain access to the Cadence SourceLink Knowledge Base will also need to complete two copies of a SourceLink Amendment to their Cadence End User Agreement (EUA).



How to Complete

End User Agreements are legal documents and must be completed with original signatures. Fax or photocopies are not acceptable. Two originals of each End User Agreement for each software package are required. End Users must complete both the front and back page of each agreement in ink.

Page 1:

EUROPRACTICE Academic Subscription Number: This is provided by the EUROPRACTICE Software Service upon applying for membership.

End User: This is the full name and address of your institution. If your institution is commonly known by an abbreviation state this in addition to the full name.

Effective Date: The date on which the Agreement is signed.

Back Page.

By: Signature of legally responsible person. Must be an original signature, in ink.

Name: Printed name (in capital letters).

Title: Position held by the signatory.

Date: The date on which the Agreement is signed.

If your Institution has an official stamp then please also include this below "date".

What Next

RAL will forward End User Agreements to the vendor for countersignature. Once countersigned by the appropriate vendor, RAL will return one original of each agreement to you.
